Responsibilities
Work with oncology providers, pharmacists, nurses, other clinicians, and Information System personnel to assure the delivery of safe, effective, appropriate and cost-efficient medication therapy to Piedmont Healthcare oncology/hematology patients. Ensures that pharmacy personnel receive ongoing clinical training to provide appropriate care. Participates and leads performance improvement activities related to medication use in designated patient populations. Serves as the primary interface between oncology providers and clinicians and the Information Systems personnel responsible for the provision of cancer and infusion care. Participates and leads committees that oversee oncology medication use. This includes, but is not limited to, Pharmacy and Therapeutics Committee and Subcommittees, Clinical Governance Councils, and Epic Operations Council.
Qualifications
Education- Graduate of an accredited ACPE school of pharmacy Required and
- Bachelor’s Degree in Pharmacy Required
- Master’s Degree in Pharmacy Preferred
Work Experience- 3 years of clinical experience Required or
- Completion of a pharmacy practice residency with two years of additional experience Required
- Oncology and Epic Beacon experience strongly Preferred
Licenses and Certifications- PHARM-R - Registered Pharmacist Licensed as a registered pharmacist in State of Georgia Required
- Board of Pharmaceutical Specialties certification Preferred
- PGY1 Residency Preferred
