The Abdominal Transplant Clinical Pharmacist provides specialized pharmaceutical care to patients undergoing kidney, pancreas and liver transplantation through all phases of transplant care, including pre-transplant evaluations, perioperative/inpatient management, discharge planning and post-transplant care.
This role involves collaboration with a multidisciplinary transplant team to ensure optimal medication management, improve patient outcomes, and maintain regulatory compliance. The Abdominal Transplant Clinical Pharmacist will join an established Transplant Pharmacy team and will rotate between the kidney/pancreas and liver services, practicing in both the inpatient and outpatient settings. This role provides the opportunity to work from home one to two days per week (dependent upon eligibility, staffing levels, and clinical needs).
-Provide direct patient care, including medication therapy management, education and counseling for transplant recipients and living donors.
-Participate in multidisciplinary inpatient rounds and contribute to clinical decision-making with the transplant team.
-Monitor and adjust immunosuppression, antimicrobial prophylaxis, and other transplant-related medications in addition to chronic disease state management.
-Conduct thorough medication reconciliation at transitions of care and assess for drug interactions, adverse effects, and adherence issues.
-Develop, implement, and monitor individualized pharmacotherapy plans for both inpatients and outpatients in accordance with institutional protocols and best practices.
-Provide transplant pharmacotherapy education to patients, caregivers, and healthcare professionals.
-Contribute to protocol development, quality improvement initiatives, and regulatory compliance for the transplant department.
-Participate in clinical research and data analysis to support optimal patient and program outcomes.
