Responsibilities

Responsible for developing, implementing and managing The Women's Heart Support Network in coordination with the Women's Heart Physician Team. Serves as point of contact for women with heart disease, inclusive of identifying and engaging women in the Piedmont Healthcare System as well as in the community. Promotes quality and engagement across the continuum of care to ensure high patient satisfaction.

Qualifications

MIMIMUM EDUCATION REQUIRED: Bachelor’s degree in nursing, social work, or other healthcare-related degree
 
MIMIMUM EDUCATION REQUIRED: Minimum of five years of experience in healthcare.
 
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Georgia LCSW or RN license based on degree
 
ADDITIONAL PREFERRED QUALITIFICATIONS: Experience with project and/or program management.
