Care Management
PRN Days
As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.
MINIMUM EDUCATION REQUIRED:
High School diploma, or the equivalent such as the General Education Development (GED)
MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in a healthcare or social services setting
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
None
ADDITIONAL QUALIFICATIONS:
Associate's degree or two years of college in human services/health related discipline preferred. Knowledge of community-based / post-acute care resources such as primary care, skilled nursing facilities, home health, durable medical equipment, and transportation.
